Your first visit to Sewee Dental Care establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. We would love for you to request any current radiographs to be sent to us from your previous dentist prior to your first appointment! Your previous dentist may require a signed Records Release Form to send us your previous dental records and radiographs. If so, please fill out the following form to send to your previous dentist and ask them to email your records to us at seweedental@gmail.com.
Insurance information
The providers at Sewee Dental Care are out-of-network with all dental insurance plans. Your insurance very likely still covers procedures from out-of-network dentists at a comparable percentage to in-network dentists. It is important to be aware of your policy’s benefits. We charge the usual and customary fees. You are responsible for payment regardless of the insurance company’s determination of these fees and the coverage of the procedure. We make every effort to provide financial options and will work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.
